    There won't be a hole because his bag was slightly open where the knife was.

    We saw Tommy closing the zip on the bag after putting the knive back in. They did not show it clearly, but it looked like there was a hole in the bag. Unless it was just a fold inthe material, regardless, hole or no hole, the knife would not have penetrated through his clothes. A very poorly shot scene.

    I thought he zipped the bag up, but not fully. Maybe it stuck or something?

    No - The bag was half open when he put the knife in. The pull for the zip was at the top of the bag, he pulled it down, all the way over till it was completely closed.
