If it is then it sounds like it might be a mental
or nervous illness rather than a phyiscal one.
But worry abt unusual physical symptoms
can cause people to behave out of character

Cain has a brain aneurysm, he goes to hospital and they confirm he does have a brain aneurysm and he needs to have surgery immediately, but he then loses it and walks out of the hospital and then goes to some river and collapses, falling into the freezing water.

It doesn't state that it was due to what zak had done to him in the spoilers, but a past blow to the head can cause an aneurysm in later life.

You need to download it and go to page 11 I think, either way what ever page it is it has a picture near the top of it of Cain looking half dead with Debbie, Charity, Moira, Vanessa and Kirin all huddled around him, so it shouldn't be that hard to find the page. Also Cain refuses to have the operation despite his loved ones begging him to have it to save his life.
