No camera people were injured in the filming of the barn fire or the explosion.
Probably no smoke because strangely, the bales of straw did not seem to be burning, just the ground and the structure of the barn. A great big beam, probably made of oak, which would take ages to burn through, falls still intact, soon after the fire started. Most amazing thing, was that after Cain ran in, the barn door was open, if only that had been the case when Moira was trying to get out.

did moira throw away the key after locking as thought ruby was emma, then in a daze
Moira's mind may be confused now, but it looked like the bolt had been there for a long time. I just cannot think of any circumstances where you would fit a padlock to the inside of a door, if there is no other way out, what purpose would it serve, except to create a dramatic situation when two women get locked in a burning barn.
