Harry's big operation day has arrived and Raf is feeling confident. However, Raf receives a shock when his brother Giuseppe materialises on the ward, steaming drunk. Before Raf can throw him off the ward, Giuseppe has some bad news to break - their mother is dead.
Raf is knocked for six, but he can recover his composure enough to tell Giuseppe that he'll come home tomorrow - today he has to stay and do Harry's surgery. He brushes off the concerns of his colleagues who claim they can find someone to replace him, but can he overcome everybody's doubts to repair Harry's face?

Meanwhile, Serena is on Keller to assess the progress of their junior doctors and is concerned that Digby still doesn't have a specialism. When pressed, Arthur panics and announces that he is interested in emergency medicine. Dom is sceptical of this panicky strategy, given that Serena thrusts Arthur straight onto a case involving an injured caver who has been rescued with a badly broken leg whilst his caving friend is still missing. To make matters worse, Arthur finds his test case too engrossing to just concentrate on the medicine.
Elsewhere, Ina is admitted to the hospital after collapsing at the bank. Mo's coldness is quickly challenged by Elliot's diagnosis - it seems that Ina may have mitral stenosis after her last surgery and there is a risk that she could bleed to death at any moment. Faced with the threat of losing the only woman she has ever called 'mum', Mo tries to speak to Ina to get further answers about her past. Will the two women forgive and forget, or will Ina take the secret to her grave?
