It's his father and Curtis is suffering from Factitious Disorder, a serious mental disorder in which the person feigns a serious illness. Both his father and doctor have been trying to get him to get treated for the disorder but he has so far refused.

So he is aware that he is not ill, and that the medication he is taking is just multi- vitamins. But when he argues with the doctor that he is serious unwell, that is what confuses me, does he actually believe he is ill at that point.
