BBC One is to screen a new three-part series this autumn after one-off drama Casualty 1906 proved a hit last year.
Cherie Lunghi and Sarah Smart will star in Casualty 1907, which features life at the Royal London Hospital in Whitechapel a century ago.
The show - set 20 years before the discovery of penicillin - is based on ward reports, autopsy records and diaries, and features real-life doctors, nurses and patients.
Joining the cast will be Lilly Allen's little brother Alfie, who will take on the role of an injured East End gang member.
Charity Wakefield - who played Miss Temple in Jane Eyre - will star as a probation nurse in the programme, which also features William Houston, Tom Riley and David Troughton.
An insider told the Mirror: "It was only our grandparents' time, yet it's a shocking, foreign world."
