There are a few lines of dialogue in this episode that seem throwaway and light-hearted at the time, but that could end up taking on greater significance in the weeks ahead. One comes when paramedic Jan jokes to her colleague Iain, “I know what a delicate flower you are.” And the second is when Iain witnesses Ruby standing up to a belligerent hospital visitor and comments to her, “My work here is done.”

As we know, Iain himself has had the worst 12 months in the workplace and while he now appears, on the surface, to be more sanguine, our fears are growing that he’s still concealing an inner turmoil. By the time the credits roll, we’re left with little doubt about Iain’s private hell. But has this realisation come too late?
