Josh arrives at the ambulance station with a new paramedic to work with Iain – it's none other than former registrar Samantha Nicholls.

At the same time, a young woman watches a house secretly from her car. When she spots someone leave, she pulls into the driveway and knocks on the front door.

A startled Iain tries to find out why Sam is now a paramedic as they travel to their first shout, but before he can discover the truth, they come across a crashed car.

Iain and Sam attend the young woman injured in the driving seat – it's the same shifty woman from the house. The car is in a precarious position on a small cliff and the paramedic duo just manage to cut the driver loose and pull her out before the car rolls down the hillside.

This causes the boot to pop open and there is a man tied up and gagged in the back. The man in the boot is far more seriously injured than the driver and his leg is impaled on part of the crashbarrier that the car had driven through. How will Iain and Sam deal with the situation?

Meanwhile, Alicia enlists Louise's help to set up Elle and Jacob, but Elle soon clocks what is happening. However, a loved-up elderly couple makes Elle wonder if she is happy living alone.

Elsewhere, during her regular morning run, Connie has to stop because she is unusually breathless.
