Serena's nephew Jason, who has Asperger's Syndrome, video-calls his aunt in panic this week, to tell her his carer, Alan, has collapsed. When Alan's brought to Holby, Serena runs tests, which reveal he's suffered a mini-stroke and needs urgent surgery. With Alan unable to work for six weeks, Serena agrees to let Jason stay with her. Jason fears Alan could die in surgery, but Serena reassures him the risks are very small.
When Alan then suffers a stroke post-op, however, Jason thinks he's dead - he accuses Serena of telling him lies and can't be around her. With Alan's condition now more serious, Serena knows he can no longer be Jason's full-time carer, but neither can she. When Serena has to tell Jason she can't look after him forever, how will he take the news?
Meanwhile, it's Cara’s first day on Darwin and she's given an initiation test – to get eight Michael Jackson songs into a conversation with Jac. But when she helps to diagnose a patient with a condition that consultant Sir Dennis had seemingly overlooked, will Cara prove just how smart she really is?
Also, when Dom learns a patient has had cancer, he tries to keep the news from Arthur, for fear of upsetting his friend, who's battling the disease himself. Will Dom realise honest is always the best policy?
